Benefit Options
Employees have two medical
plans for calendar year 2012 - the
Aetna Health
Fund (AHF) and the
Choice POS II
medical plan. The
following tools can help you decide which plan is best for you and your
family.

|
Family Medical Leave Act (FMLA) The Family and Medical Leave Act (FMLA) of 1993 provides up to 12 weeks,
in any 12 months, of unpaid, job-protected leave for birth or care for
newborn of the employee, serious health condition, adoption and more.

Short Term Disability
Disability benefit available to employees of the Sarasota County
Commission (excluding IAFF), Clerk of the
Circuit Court, Property Appraiser and Supervisor of
Elections. Provides income protection for employees out of work due to
an extended illness or injury.
|
Long-Term Disability (LTD)
LTD is offered through Sun Life of Canada. Sarasota County pays for a 40 percent
benefit for employees (60 percent for "At-Will" employees.)
Non-At-will employees may increase their LTD
coverage by 20 percent for a total benefit of 60 percent.

Flexible Spending Accounts
Sarasota County offers both Medical and Dependent Care Flexible Spending
Accounts (FSA). These accounts allow employees to save money by placing
pre-tax contributions into accounts for medical and dependent care
expenses. Note: the medical spending account can be used for medical
expenses for you, your spouse and children. The Dependent Care Flex
Account is for day care expenses only. The FSA program is administered
by Aetna.
Sarasota County contributes 25 percent to participants' Dependent Care
FSA to assist with childcare expenses, subject to the $5,000 annual
maximum contribution established by the IRS. For example, if an employee
elects to contribute an annual amount of $4,000, the county will
contribute $1,000, for a total annual amount of $5,000. Sarasota County
also contributes 5 percent toward the Medical FSA, subject to the same
annual maximum above.

Cancer and Specified Disease Insurance
Plans
Sarasota County offers cancer insurance through Allstate beginning in
2011. The policy will be guaranteed issue. An application must be filled
out for all new policies or changes to an existing
policy. Employees who currently have Hartford and AIG cancer policies may
continue those through payroll deduction, however no
increase in coverage is available for those policies and for 2011 there
is a 17 percent increase in premiums.

Identity Theft
Identity Theft coverage will be offered through InfoArmor. Coverage
includes $25,000 identity fraud reimbursement policy. Coverage is only
$3.67 per pay for employee. Family coverage is $6.44 per pay.

Long Term Care Insurance
Long-Term Care insurance is offered through UNUM as an employee-paid
benefit and is available via after-tax payroll deduction.

Life and Accidental Death
Insurance
Employee Group Term Life and Accidental
Death and Dismemberment (AD&D) Insurance is provided as part of your
Cafeteria Plan benefits.
Group Term Life Insurance
The policy pays a designated beneficiary (or
beneficiaries) a fixed sum of cash in the event of a covered employee’s
death. This insurance is purchased on a year-by-year basis and
accumulates no cash value. The Group Term Life Insurance program also
includes a special Accidental Death and Dismemberment provision, which
doubles the amount of basic benefit if the death is the result of an
accident or the payment if dismemberment occurs.
For eligible employees, the County will pay for an amount equal to one
times the annual salary, rounded up to the next $1,000. For At-Will
(Management) employees, the County will pay for an amount equal to two
times the annual salary, rounded up to the next $1,000. The minimum
benefit is $10,000. Coverage becomes effective 30 days following the
date of hire.
Optional Coverage
Additional coverage equal to one or two times your annual salary can be
purchased. If optional coverage exceeds $50,000 as a new hire or if
coverage is increased during open enrollment, an Evidence of
Insurability form must be completed. These forms are available below.
The optional coverage will not go into effect until the insurance
carrier approves the application.
The first $50,000 of coverage (including Core and optional coverage) can
be paid with pretax deductions. If total coverage exceeds $50,000 (core
and optional), the participant is subject to additional taxes.

Life Insurance

Term life
insurance worth the employee's salary is county paid. Up to double
the employee's salary is available through Sun Life of Canada. Sun
Life also offers dependent life insurance.
Note: any
increase in coverage requires an Evidence of Insurability
form.
- Universal Life insurance is available through
Allstate. This option
includes various coverage amounts and dependent coverage. Universal life insurance requires an application.
Allstate Flexible Premium Universal Life (Cash Value)
This Allstate Group Universal Life Policy provides great
flexibility of life insurance benefits and growth of cash values. It is
available for employees on a guaranteed issue basis. The plan builds
cash values at a current interest rate of 5.25% (Guaranteed not to go
below 4%) and is portable upon retirement or separation of service. This
coverage is employee-paid and is available via after-tax payroll

Allstate Group Accident Plan
Accidents most frequently happen in places where we spend
the most time – in the home or during sports or leisure activities.
Approximately 22 percent of injury visits to hospital emergency rooms or
urgent care clinics are the result of a sports-related activity for
persons between the ages of 5 and 24. Our medical insurance plans
provide coverage for related expenses but require co-payments,
deductibles and co-insurance. The Allstate Group accident plan
provides over 30 different benefits should you or a covered family
member have an accidental injury and require medical care. The plan
pays in addition to any medical plan coverage for injuries such as
dislocations and fractures, as well as reimbursements for follow-up
care , hospital admissions and surgical care. This coverage is
employee-paid and is available via pre-tax payroll deduction. Consult
