| The Advantica Plan
offers routine eye examinations and corrective lenses (if needed) every
12 months. Frames are provided every 24 months. Some restrictions may
apply on the type of lenses and frames that are covered at full cost.
There is a $10 deductible for each covered eye examination and a $15
deductible for covered frames and/or lenses. Contact lenses can be
purchased in lieu of frames and lenses and the plan will pay $150 toward
lenses less applicable copay. Although coverage is not restricted to
the Advantica provider network using an Advantica provider will ensure the maximum
reimbursement available. If a non-Advantica provider is used, benefits are
based on a fee schedule and the participant may not receive the same
level of coverage offered through an Advantica provider.
To obtain vision services you must show
your Advantica card to the vision care provider. Forms and lists of
network providers can be obtained on the
Advantica's
Web site or
by contacting Advantica at 866-425-2323.
